Asiri Rathnayake a1d7d2ffb8 [libcxx] Don't assume __libcpp_thread_t is an integral type 
We have already refactored the underlying platform thread type into
__libcpp_thread_t, but there are few places in the source where we
still assume it is an integral type.

This patch refactores those points back into the threading API.

libc++ Documentation
====================

The libc++ documentation is written using the Sphinx documentation generator. It is
currently tested with Sphinx 1.1.3.

To build the documents into html configure libc++ with the following cmake options:

  * -DLLVM_ENABLE_SPHINX=ON
  * -DLIBCXX_INCLUDE_DOCS=ON

After configuring libc++ with these options the make rule `docs-libcxx-html`
should be available.
