Alan Stokes eae2ff9831 Fix OWNERS issues ...

Switch to a bug template that assigns the correct group.

Consolidate into one file to make it easier for humans as well as
machines to figure out the ownership.

Remove me from per-file ownership; no longer relevant, causes
confusion.
